Step 5: Form the team

Experts from the Interprofessional Education Collaborative recommend you engage diverse health care professionals who complement your own professional expertise, as well as associated resources, to develop strategies to meet specific patient care needs.

The last part of that sentence is the most important. Patient care needs should determine which professionals form the team. As the complexity of care increases, the size of the team may increase and vice versa. Teams may also be established and expect to see certain types of patients or ad hoc in order to respond to emergent or urgent situations.

Topics covered during this step include health conditions requiring a team, team formation strategies, and differences between established and ad hoc teams.
