About Us

The mission of Education to Practice (E2P)  is to decrease everyone’s interprofessional collaboration learning curve so they can achieve the Triple Aim plus improve the work life of clinicians and staff. Check out our experience helping partners achieve this Quadruple Aim.
What is interprofessional collaboration? A process for communication and decision making that enables the separate and shared knowledge and skills of different care providers to synergistically influence the care provided through changed attitudes and behaviors, all the while emphasizing person-centered goals and values (Health Canada, 2010).

Our Experience:
Start a statewide interprofessional dialogue Convened recurring workgroups involving 44 individuals from 23 organizations Many in Michigan expressed interest in the topic, but only E2P could serve as a neutral convener
